Who is Colfax Health & Rehabilitation Center?
Colfax Health & Rehabilitation Center (formerly known as Area Nursing Home) has served our community for over 46 years with high quality long term care services. It was started by community members who saw a need to provide care to their family, neighbors and friends. Shares of stock were sold to those who supported this vision. Throughout our history we have remained owned and operated by community members. In 2010 the stockholders voted overwhelmingly to convert ownership from a stock corporation to a 501(c)(3) charitable organization to ensure their home would always remain a community operated entity. Through the years our mission has remained the same…To embrace our residents, their families and our staff in a place that feels like home. We are committed to providing exceptional, individualized care with compassion, pride and respect.

Colfax Health & Rehabilitation Center is asking for your support of this project and all who embrace this significant opportunity. A successful campaign of volunteer workers and generous donors will make this vision a reality for the greater Colfax community ensuring the legacy of care continues for generations to come. The fundraising goal for the project $500,000; a relatively small amount related to the overall 12 million dollar project. Please consider a three-year pledge. Why is a Capital Campaign necessary?
Colfax Health & Rehabilitation Center is financing this project through the US Department of Agriculture, office of Rural Development. As part of the financing process, we are required to raise funds from the community as a contribution. Proceeds from the campaign will ensure this project’s financing plan is sustainable and affordable.

How much will the facility cost?
Overall cost of the project is 11.75 million dollars.
What will you do with the old facility?
The Mayo Clinic will remain in the current facility and account for its primary use. The board of directors hopes to find suitable and complimentary use for the remainder of the facility and will study these options closer to the time we move out.
Will this building cause my taxes to go up?
No. Colfax Health & Rehab is funding this project through loans from the USDA and community fundraising efforts.
Why are you building a new facility?
Our current facility is unable to accommodate the services our community needs, specifically assisted living. There are several other costly updates that were needed and required at our existing site in order to maintain its operation as a nursing home and provide for the changing needs of seniors. By taking advantage of some special programs offered by the state of Wisconsin we were able to obtain substantial assistance in building a new campus that expands our services to the community and provides a much more home like, efficient and state of the art facility to provide these expanded services.
How will I be recognized?
Colfax Health & Rehabilitation Center is extremely grateful for the tremendous support we have received from the community in our efforts to raise funds for the new facility. All donors of $1000 and above will be recognized for their contribution on a permanent display in the lobby of the new facility. Donor recognition gift levels range from $1000 to 250,000. Donor’s names will be noted under the appropriate gift level. Please see the additional information provided regarding naming opportunities that are available for designated areas of the facility.
